What it is
Overload of the ECU tendon or instability within its sheath on the ulnar side of the wrist, often after forceful rotation or in racket sports.
Typical clinical symptoms
- Ulnar and dorsal wrist pain
- Pain with resisted extension and ulnar deviation
- Snapping of the tendon during pronation and supination
- Pain with racket grip, lifting or weight-bearing through the hand

When is medical assessment needed?
Deformity after trauma, a finger that cannot actively bend or straighten, progressive loss of sensation or strength, a cold or pulseless hand, an open wound, and redness with fever require prompt medical assessment.
