What it is
Irritation or entrapment of the sciatic nerve in the deep gluteal space. It is broader than “piriformis syndrome” and lumbar sources must be excluded.
Typical clinical symptoms
- Deep buttock pain
- Pain or tingling spreading down the back of the thigh
- Aggravation with prolonged sitting or positions that tension the region

Trauma with deformity or inability to bear weight, a very painful hip with fever, a child or adolescent with sudden limping, progressive neurological weakness, bladder or bowel change, and unexplained pain with systemic symptoms require prompt medical assessment.
