What it is
Irritation or compression of the deep radial nerve in the proximal forearm. Radial tunnel syndrome primarily causes pain; motor involvement of the posterior interosseous nerve causes weakness.
Typical clinical symptoms
- Deep lateral elbow pain, usually below the epicondyle
- Pain with resisted supination or middle-finger extension
- Numbness is usually absent in radial tunnel syndrome
- Weak finger or wrist extension when the motor branch is involved

When is medical coordination needed?
Deformity after trauma, immediate inability to move the arm, severe pain with a tense forearm, a cold or pulseless hand, progressive numbness or weakness, and redness with fever require prompt medical assessment.
Descriptions are for orientation. Diagnosis follows history, clinical examination and, only when indicated, imaging or laboratory tests.
