What it is
Lowering of the medial arch during loading that may be a normal, asymptomatic variant; it becomes clinically relevant when associated with pain or limitation.
Typical clinical symptoms
- Fatigue or pain in the arch, heel or ankle
- Difficulty with long walks or sport
- The arch reappears when unloaded in flexible flatfoot

When is medical assessment needed?
Deformity or an open wound after trauma, inability to bear weight, a cold or pulseless foot, progressive numbness or weakness, a red hot joint with fever, and a neuropathic foot that becomes suddenly hot and swollen require prompt medical assessment.
